Dear patient or conglomerate of this office,

We value your business and respect the privacy of your information, which is why, as a precautionary measure, we are writing to let you know about a potential data security incident that [may involve/involves] your personal information.

On the night of May 1st, 2017 Electronics were stolen from my (Dr. Kelly’s) car that included; a work/ personal Laptop and hard drive, as well as other personal items in my car. The Laptop and hard drive had all of my personal photos and data as well as and more importantly your patient intake files that had been scanned and filed in folders that were all under password protection. I have no reason to believe these petty thieve/s were intending on using information but; more and likely they would be erasing the drives to resell them for their electronic cash value. (I make this statement because I also had documents that were more visible including renewal of passport information that had my social security number on it, and they left it in my car).

The potentially accessed data [may have included/included] your personal information such as your intake forms with your address phone and email information on it, and insurance EOB’s (Explanation of benefits). Luckily and purposely, I do not take Social security numbers and if I do I never keep any records of it electronically. To my knowledge, the data has not been accessed, but it is not known if they will be tech savvy enough to break my passwords or even care that there is data on the devices.

I would like to sincerely apologize for having to inform you of this. Please let me know if you find out any reason to suspect your information is being used. If so we will take more actions to try to track down the culprits.

Thank you for your trust in me to do everything I can do to find who’s at fault. I do wish that these thieves are simple minded and money focused. Please forgive me for having my work electronics with me. Though, I am very cautious and did all I am required to do to protect our information. I still feel horrible for the vulnerable position this can put you in. Feel free to contact me with any questions or concerns.
